Thank you for completing this required form to participate. A parent or legal guardian must complete this form on behalf of minors.

Adult chaperones are required with youth ages 12 and under.

Please Note: Participation in the Car Smash activity is limited to ages 13 and up, and all minors must have a parent/guardian present. Closed-toe shoes (not crocs) are required. Safety gear will be provided.

Acknowledgement

I consent for my own and/or my youth's participation in Nerf & Nachos at Life Church AG in Garrettsville, Ohio. I understand that participation is voluntary and acknowledge the inherent risks involved with recreational activities, including but not limited to slips, falls, collisions, cuts, bruises, strains, and other injuries. On behalf of myself and/or my youth, I voluntarily assume those risks and agree to release and hold harmless the Life Church AG, its employees, volunteers, and event leaders from claims arising out of risks associated with participation in these events.

I and my youth will follow all rules, treat others respectfully, and follow directions from event leaders. I understand that failure to follow rules and directions may result in removal from an activity or the event.

Should a medical emergency arise, I understand the church team will make every reasonable effort to notify me and/or my emergency contact and understand church staff or volunteers may need to obtain emergency medical treatment.

I understand that event photos, videos, or likenesses may include myself and/or my youth and will be the property of Life Church AG. I grant permission for Life Church AG and/or Life Church's Awakening Youth to use these, with or without names, for promotionals including but not limited to website content, social media, or printed materials. If I have any concerns or need myself or my youth to be excluded, I will text/call Melanie Coleman at (419) 924-8299 promptly.
